To replace a lost or stolen ID/Program card, complete the attached click submit. ID/Program cards will be processed and available for pick up within two days. In the meantime, a temporary ID will be given to to the student. Until receipt of the permanent ID/Program card, certain privileges, such as eating lunch outside of school, will be suspended. Please refer to the official school ID/Program Card Policy for further information. Students may submit the form ONLINE by clicking SUBMIT and then pick up their ID in Room 209 by paying the $3 fee; or submit application and fee in Room 209 for processing.
For School year 2020-2021; Blended Freshmen will be issued IDs upon coming to school when called individually to take photos and should not use this form.
